ALT leaks into the blood when liver cells are inflamed or damaged. Much of that damage is oxidative stress, the internal “rust.” Your main defense against it is glutathione. NAC hands your liver the exact building block to make more. Milk thistle, by contrast, mostly acts as an outside antioxidant and cell-membrane shield — it can shield cells, but it doesn’t restock the glutathione those cells actually run on. NAC restocks what they actually run on.
